and will be required to sign in and out of our site each time you attend.

This applies even if you are just popping in for a quick visit.

What you need to do:

  1. Ensure you have your proof of flu vaccination ready to submit.

  2. Click the "Register Here" button (below).

  3. Fill out the required information and read through the required information.

  4. You will be emailed an ID card. This can either be printed or saved directly to your phone. You will be required to scan this card each time you enter and exit one of our sites on the iPads provided.
